Client education and marketing program

What the program entails

By participating in the Client Education and Marketing Program, you authorize Foresight Mental Health, PLLC (“Foresight”) to share with you information about products and services provided by organizations other than Foresight that may be of interest to you, such as health education materials prepared by third parties and applications that can integrate with Foresight applications, and other health related products and services. 

 

This authorization permits Foresight to use your demographic information and the name(s) of your treating physicians in order to include you on mailing lists and to use information about your health care, to identify programs and initiatives that are likely to interest you. 

 

Foresight does not receive any compensation for sharing your information. Foresight will not provide this information to unrelated parties for their own marketing and will not use this information for telephone solicitations. Foresight will not sell your information to third parties for their own marketing activities. Specially protected information, such as protected health information, will not be used for marketing purposes without specific permission from you.

 Program protection and rights

• We will not share any information with third parties pursuant to this authorization.  

• You have the right to revoke or receive a copy of this authorization at any time by providing written notice to info@foresightmentalhealth.com. 

 

This authorization shall be in force and remain effective until you are no longer an established client at Foresight, unless you revoke at any time prior. 

 

Your treatment, payment, enrollment or eligibility for benefits will not be conditioned on whether you choose to participate in the Client Education Program. You can learn more about Foresight's privacy policies by viewing our Notice of Health Information Privacy Practices.
